To become a new patient, call our scheduling department for more information. Call 317-572-2240, and press option 2.

For patients living with chronic pain and have been referred for a consultation, we will need to obtain all relevant medical records prior to scheduling. You may need to sign a "Release of Information" form from prior treating physicians. They can fax records directly to us on our secure fax line: 317-572-2235

Insurance
We accept most major insurance plans, Medicare, and Tricare plans. We can also see Workman's Comp cases, with authorization. We do not accept Medicaid, HIP, or Humana plans. Please call our scheduler for any questions about insurance coverage.

Cancellation Policy
If you cannot come to your scheduled appointment, please call our scheduler to reschedule within 24 hours to avoid a cancellation fee.
